If you choose to issue certificates or evidence of insurance, JSA bears no responsibility for the accuracy of such documents.  If JSA receives a certificate or evidence of insurance issued by a retail agent, we will not check the document for accuracy and it will not be deemed as a request to modify coverage.  Any request to modify coverage must be submitted in writing to JSA for advanced written approval.  Submission of a certificate or evidence of insurance to our office does not constitute approval of the certificate or evidence of insurance.
