Endorsement Request Process for Stonewood Transportation Program

Note there are two different forms to choose from: Truckers or Public Auto/Special Types.

1. If this is a request to add a unit to a Stonewood Policy, you will need to fill out one of the two Stonewood endorsement request forms located on the tabs at right.
2. With either form, Stonewood requires that you provide ALL answers to ALL the questions.
3. If you also need to make a change to a cargo or physical damage policy for the same insured you can include that in this form.
4. If you have your own form that you generate out of your agency management system you can just attach it below as long as it answers all the same questions we ask on the Stonewood form.
5. If you choose to use the E-sign document, it is automatically sent to us after you complete and click DONE.
6. If you choose the pdf form, download it to your computer, fill it out and email it to endorsements@jsausa.com or through the portal on this page. You can even choose to fax the form back to us at 1-828-262-0754.

Once you submit the endorsement request to us for consideration, you’ll receive an acknowledgment letting you know that we have received your request.
