A Checklist for Submitting a Transportation Quote
- Full insured name
- Both mailing and garaging addresses
- DOT and MC numbers
- Brief submission narrative including lines of coverage needed, target pricing and how we can help you write the account
- Limits and coverages desired. Auto Liability, physical damage, cargo, GL etc.
- Number of years the insured has been in business.
- Prior carrier and loss history for all lines of coverage requested. Loss runs, if available.
- Drivers List including name, date of birth, driver’s license number, state, number of years CDL experience, violation/accident history for the past three years. MVRs are great if you have them but not required.
- Full Unit Information, which includes year, make, model, VIN, actual cash value, whether the vehicle is owned, leased, or an owner/operator.
- Radius the insured operates on. Include farthest states and any major cities frequented. IFTAs if available.
- Percentage breakdown of commodities being transported.
- Disclose any certificate or contract requirements.
